Long Term Conditions - Annual Reviews

If you have a long term health condition, we encourage you to arrange a yearly review around the date of your birthday. This makes it easier to remember. Please also start a review if you have not had a review in the last 12 months. We will also send a text message and/or letter reminder in your birthday month.

The best way to start this review is by using the Medlink service. This allows you to complete some of the information about your health we need from the comfort of your home and in your own time.  We will then arrange any further appointments or tests required.

If you feel unable to complete the online forms please contact the practice on 0141 800 0840 to arrange for your annual health check.

Multiple Conditions

If you have two or more of:

  • Asthma
  • COPD (also known as emphysema or chronic bronchitis)
  • Coronary Heart Disease (eg angina or previous heart attack)
  • Diabetes
  • Epilepsy
  • Rheumatoid Arthritis

Heart Disease (coronary)

For example angina, previous heart attacks or Ischaemic Heart Disease 

Mental Health

Patient who are have severe and enduring mental health issues such as schizophrenia will be offered an annual physical health check. 

The mental health is reviewed by Community Mental Health Teams in Springpark, Shawpark and Belmont (Old age) Resource Centres

Learning Disability

The health board will contact patients registered with a Learning Disability for their annual health check and review. 

Rheumatoid Arthritis

The specialist teams at Stobhill Hospital and Glasgow Royal Infirmary will usually monitor arthritis.
