New Patient Registration
Please ask our receptionist for a registration form. She will help you to complete it if necessary. Or you can download a registration form from our website, fill it in and bring it along with you.
When you come in to the practice for the first time, you must bring with you an item of personal identification with a photograph on it, e.g. a passport, driving licence or bus pass. You must also bring an item with your current address on it. This item must be dated within the last 6 weeks.
Once you have completed a registration form, all patients aged 5 years or more will be given an appointment with the Practice Nurse or Health Care Assistant for a New Patient Health Check. You must attend this appointment to complete the registration process. The receptionist will also give you a practice leaflet and a container for a urine specimen which you should bring to your appointment together with your medical card, if you have one.
Medical treatment is available from the date of registration. If you need to see a GP or Nurse before the date of your New Patient Health Check, please click on the Appointments button above for information on how to arrange this.
